Dismissal Form


  • **If there are differing methods for one or more of your students, please fill out this form for each student with differing dismissal methods.**



  • Please complete this form to indicate how you would like your child to be dismissed daily. If your dismissal arrangements should change, please complete a new form. If you have a one day change, you must complete the small forms located in the front office and give it to your child’s teacher during morning drop off.

    If you need to pick up your child early, please notify Liz Harris, the School Secretary as early as possible. Dismissal changes cannot be made after 2:55 pm. No one will be allowed to enter the building after 2:55 pm to allow guides time to prepare for a smooth dismissal.

    If a last minute emergency should occur, please contact Liz for assistance.

    Please note that the school is not responsible for tracking which days students walk and which days they may not. By signing this agreement, you are indicating that you are aware of this and that once your child leaves the front door, the school no longer assumes responsibility for your child.
