Before and After School Care Letter & Consent


  • ** We ask that only families requiring this service fill out this form. For families requiring this service with multiple students, please fill out a copy of this form for each of your children.**


  • Dear Parents, we are happy to have your child participate in City Garden’s Before & Aftercare School Program. This letter includes details regarding the program.

  • Who Can Participate?

  • Before and After school care will be offered to children in both the City Garden Primary (Preschool) Program and City Garden Montessori Charter School. Though children may be divided into age groupings for some activities, children of all ages will be part of the same program and will be together for some portion of the time.

  • Beforecare Schedule/Hours

  • The before care program will operate from 6:45-8:00 a.m. each school day. This program includes breakfast and is supervised by City Garden employee’s and staffed by SLU students. The cost of before care is $7 per day per child.

  • Aftercare Schedule/ Hours

  • The aftercare program will operate from 3:45-5:30 p.m. each school day. Please pick up your child promptly by 5:30 p.m. For each minute past 5:35 p.m. that you are late, you will be charged $5 per minute. Thank you for your cooperation.

    Cost

    City Garden’s after-school care program is a tuition-based program. Parents are required to pay tuition the week prior the week of service. Tuition will not be reimbursed for days the child does not attend due to illness or other circumstances.

    The cost per child will be as follows:
  • 1) 5 days per week=$50 (plus an additional $5 on early dismissal days)
  • 2) $12 per day for less than five days per week if you pay in advance.
  • 3) $14 per day if you do not sign up and pay in advance.
  • 4) $7 per day if you pick up by 4:15pm.
  • 5) There will be a 3% convenience fee for all payments submitted by PayPal. This is to cover the processing cost.
  • 6) Pick-up after 5:35pm: $5.00 per minute per child until the child(ren) are picked up.

  • If you need assistance and can demonstrate financial need, please contact Tiffany Whitlock Murphy (tiff@citygardenschool.org) for a financial assistance application.

  • If you need care for less than five days per week, please indicate the days needed on the Registration and Consent Form. We will do our best to accommodate all families’ needs.

    We will reserve the slots requested for your child and ask that you commit to pay the tuition for these slots. If your schedule must change, please notify us at least two weeks in advance. Otherwise, you will be charged the tuition whether your child attends.

    Care is available from 1:00pm to 5:30pm on early dismissal days. The cost for early dismissal days is the cost of daily or weekly care plus an additional $5.

  • Late pick-up from After School Program:

  • Your child must be signed out of the Aftercare Program by a parent or other designated person by 5:30 p.m. If your child remains at the program after 5:30 p.m., you will be charged additional fees:

    Pick-up after 5:35 pm: $5.00 per minute per child until the child(ren) are picked up.

    Please note: If the child is not picked up by 6:00 p.m., it may be necessary to call the police and/or the Division of Youth and Family Services for assistance. Repeated late pick-ups (five late pick-ups) will be cause for dismissal from the Aftercare program. Families will be notified after 3 late pick-ups.

  • Late payment for before and afterschool programs:

    The before and aftercare programs must pay for themselves through the collection of tuition. Therefore, is it imperative that payment be made in a timely manner. Failure to do so puts the program at risk. Since the 2017-18 school year, City Garden has been instilling the following Late Payment Process

    30 days past due, a warning letter is mailed home to family alerting family of past due amount.

    45 days past due, a second warning is mailed home and payment is due by a specific day. Failure to remit payment will result in your child no longer being eligible for the program. Financial aid will be revoked.

    60 days past due, a third letter is mailed home alerting family that all before and aftercare privileges have been revoked until payment is received and/or a payment plan has been created.

    120 days past due, balance is handed over to a collection agency who will take over seeking balance due. Any charges for use of collection agency or legal fees are the responsibility of the family.

    Inclement Weather

    If there is a late start due to inclement weather, there will be NO Beforecare. We will do our best to let families know by 6am. If school is dismissed due to weather, there will be no aftercare. We are unable to offer any care on days the school is closed. We do not offer childcare services on parent conference days.

    Pick-Up

    Pick-up for the after-school care program will be in the gymnasium. You must sign your child out on the attendance sheet each day before leaving. Please have a photo ID (i.e. driver’s license) available if asked. We will be checking EVERYONE’S ID for the first few weeks. A list of adults allowed to pick up your children must be on file in the front office. This list should include the full names and phone numbers of adults.

  • What Will Children Do at Before-School Care?

    During the morning program children will be invited to play games, read books, and do arts & crafts. Breakfast will be served at 7:45am. At 8am the children will be dismissed to their classrooms.

  • What Will Children Do at After-School Care?

    The programming for our after-school care program will be relatively low-key and simple. The children will eat a snack and unwind from their school day, have time outdoors, play games, do arts and crafts and (for the older children) have time to work on homework.   • Behavior Expectations

    City Garden reserves the right to dismiss a student if his or her behavior is overly disruptive. All students are expected to adhere to the same behavioral expectations of their classrooms. The before/ after-school care program is an optional program meant primarily to provide child care for families who need it. Thus, staff will not be able to spend inordinate amounts of time responding to challenging behaviors.

  • Payment

    Please turn in payment the week prior to utilizing the program. Place payment and envelope in the aftercare mailbox located at the reception area. Please include parent name, student(s) name(s), which dates you’re signing up and paying for on the outside of the envelope.


  • For the 2020-21 school year, my child will be attending the before/after-school program:



  • I agree to the above terms and charges for the before and aftercare programs. I will submit payment for before and aftercare in advance each week. I understand that I must pick up my child by 5:30pm each day, and that I will be charged $5 per minute for every minute past 5:35 that I am late. I agree that all information on file at my child’s school (including but not limited to emergency/medical information, up-to-date immunizations, person(s) authorized to pick-up my child, permission to photograph/videotape) also apply to the after-school care program. I agree to not send my child to before/after-school care if my child is ill and understand I must pick up my child should they become ill while in attendance. I understand that if I fail to make timely payments, before and aftercare privileges will be revoked. I understand that if I have been awarded financial aid for either program and fail to make payments in a timely manner, I am no longer eligible for financial aid services.
