Welcome to the City Garden Montessori School Application Page! The application process is easy and has three separate parts, The Initial Application Form, The Application Upload Form, and an Application Payment followup e-mail in order to process your application. Please review the content below, and click the button at the bottom of the page to fill out the application.
1) CGMI:TEP Initial Application Form
For your initial application you will provide basic demographic information.
Note: If you have an organization that will be sponsoring you for training you will need to be prepared with the following information as part of your application process…
1. Organizations name
2. Name of person in organization sponsoring you for training
3. Mailing address of the organization
4. Phone number of your sponsor
5. Professional e-mail of your sponsor
2) CGMI:TEP Application Upload Form (e-mailed to you following receipt of your initial application)
You will need to gather, scan, and upload the following items:
• Photo of yourself for your profile
• State Issued Photo ID
• Current AMS or AMI Credentials (if applicable)
• Current State Issued Teaching License
• College Transcripts
• Professional Resume
• Contact Information for Three Letters of Recommendation*
• Personal Essay*
• Signed Tuition Agreement*
*Instructions are included in the Application Upload Form you will be sent after submitting your Initial Application Form. You will be able to return to it to upload as you gather items.
3) Once you submit your forms and requested materials, you will be sent a PayPal link for the $100 Application Fee. Applications submitted without this fee will not be processed. Click the button below to begin the application process.
